To become a patient of StoneCreek Dental Care, you will need to fill out ALL the necessary paperwork and present it to the receptionist on the date of your first visit. For printable copies of all paperwork, click here. And to set an appointment, either call your local office or fill out the appointment request form.

If you need to cancel an appointment, call the location where you are scheduled at least 24 hours prior to your appointment time. You can find the contact information for each of our offices in the Locations tab above.

If your teeth are relatively healthy and you have no problems with decay or gum disease, you should visit your dentist at least every 6 months for a proper checkup and cleaning.
